Completing the application:
- Application forms may be obtained from the ABCP Administrative Office and may be photocopied provided all pages are clearly legible, or you may print the application found here.
- The application forms must be complete and must be received by the application deadline.
- Requested copies of all documents must be attached to the application and received by the deadline.
- Incomplete applications will not be reviewed. The applicant is responsible for submitting an application complete in all aspects.
- The Credentials Committee, at its sole discretion, may request supplemental material to clarify an application; however, it will not delay the review of an application if it is complete.
- The applicant must provide the original and an electronic copy of the application and all requested documentation materials (including publications).
After completing the application:

Note: If an overnight courier is employed, the best overnight courier to use is the U.S. Postal Service's Express Mail delivery, since many overnight couriers do not deliver to a post office box.
ALL CONTACT between applicants and Board members regarding the application process will be through the administrative office in San Antonio, Texas.
Application checklist:
The following is needed to assure a complete application:
- Application form, including an identification form with a recent photograph.
- A reproduction or copy of documentation of degrees. (notarized)
- Documentation of primary ABMS Board certification, if applicable.
- Letters of recommendation (submitted directly to the Administrative Office of the ABCP, addressed to the Credentials Committee.)
- Bibliography.
- Appropriate fee(s) as indicated on the application. All remittances must be made only by check or money order for United States currency, drawn on a U.S. bank, made payable to the American Board of Clinical Pharmacology, Inc.
APPLICATION DEADLINE: The deadline for receipt of application is APRIL 30, 2009.
Applications received after this date must include the late fee. Incomplete applications will be returned without review.
Applications will not be accepted after APRIL 30, 2009.
